Title: Patient Support Advocate (Bilingual, Spanish)

Compensation: $22/hour
Position Summary:
Rx Outreach is seeking a compassionate and mission-driven Patient Support Advocate to join our patient call center team. In this frontline role, you will provide direct assistance to patients, caregivers, and healthcare partners across the country, helping them access affordable medications and navigate Rx Outreach’s pharmacy services. This role is ideal for someone who thrives in a fast-paced, service-oriented environment and is passionate about making a difference in people’s lives.
What You’ll Do:
- Provide empathetic, professional assistance to patients via phone, email, and online messages, addressing questions about prescriptions, program eligibility, order status, pricing, and more.
- Support patients and partners through complex medication access needs, offering clear guidance and escalating concerns to the appropriate internal teams when needed.
- Ensure timely and accurate data entry across systems, including patient records, orders, and partner information.
- Help patients troubleshoot account access or documentation issues and proactively follow up to ensure resolution and satisfaction.
- Educate patients on Rx Outreach’s services and mission, empowering them to make informed choices about their health.
- Collaborate with team members to support a consistent, high-quality patient experience across all touchpoints.
- Operate within a mission-driven, performance-oriented environment, meeting benchmarks related to responsiveness, patient satisfaction, and data accuracy.
- Identify common challenges or trends in patient concerns and recommend solutions to improve service and efficiency.
- Actively participate in a culture of learning and feedback—both giving and receiving—with the goal of continually improving our processes and your own development.
What You’ll Bring:
- Experience in a customer service or call center role, especially within a healthcare, nonprofit, or pharmacy setting.
- Comfort working across multiple channels (phone, email, online forms) in a fast-paced environment.
- A compassionate, problem-solving mindset with the ability to handle sensitive or urgent patient issues.
- Strong communication and organizational skills, with attention to detail and follow-through.
- Familiarity with patient support systems, pharmacy operations, or healthcare navigation (experience with Medicare patients a plus).
- Experience working in a mission-focused or metrics-driven environment.
- Willingness to learn, grow, and contribute ideas for improving the patient experience and internal systems.
- Comfort with remote work tools and a stable internet connection for occasional remote work opportunities.
- Bilingual (English/Spanish) required.

Required Education & Experience
- High School Diploma or its equivalent
- Able to get Pharmacy Technician Licensure or provide proof of current license

Title: Community Health Worker
Department: Dellwood Community Pharmacy
Reports to: Pharmacist in Charge, Dellwood Community Pharmacy
Classification: Non-Exempt
Position Summary
A Community Health Worker (CHW) with a Pharmacy Technician role bridges medication and health services by assisting pharmacists with dispensing, educating patients on medication use and adherence, connecting them with community resources to address social determinants of health (like transportation or cost barriers), and providing follow-up care. They perform traditional pharmacy technician duties such as inventory management and insurance processing while also conducting home visits, health screenings, and health education to improve patient outcomes and access to care.
Key Responsibilities
Patient Education & Support
- Educating patients on medication use, dosage, potential side effects, and providing practical tools for medication adherence.
Community Resource Navigation
- Connecting patients to community resources to address barriers to care, such as transportation, food, housing, and financial assistance.
Health Promotion & Screening
- Supporting public health initiatives, performing health screenings (i.e. for blood pressure or chronic conditions), and offering education on lifestyle changes.
Medication Dispensing Support
- Assisting pharmacists with stocking, assembling, and labeling medications, as well as managing inventory and processing prescriptions and insurance claims.
Follow-Up & Outreach
- Conducting follow-up home visits, calling patients to schedule appointments or provide reminders, and documenting patient assessments according to program protocols.
Collaboration
- Building relationships with local community members, GP practices, pharmacists, and other healthcare providers to support patient care.
Qualifications
- Community Health Worker (CHW) certification required.
- Minimum of 2 years of CHW experience required.
- High school diploma or equivalent required; associate’s degree preferred.
- Ability to obtain Pharmacy Technician Licensure or provide proof of current license.
- Prior experience in team leadership or supervisory role strongly preferred.
- Strong knowledge of Rx Outreach programs (or willingness to quickly gain expertise).
- Excellent verbal communication, problem-solving, and documentation skills.
- Ability to handle sensitive information with professionalism and compassion.
- Proficiency with basic computer applications such as Windows, Outlook, and other standard office software is required.

Physical Demands
This position requires the ability to remain at a workstation for extended periods while assisting patients by phone, responding to emails, and participating in virtual consultations or team meetings. The role also involves periodic movement within the pharmacy to collaborate with staff, access patient records, retrieve medications or supplies, and utilize office resources. Adequate vision and hearing are essential to accurately review prescriptions and program materials, ensure safe and effective communication, and engage with patients, providers, and partners in person, by phone, or virtually.
